The Cardiac Rehabilitation Continuum: An example of menu-based service provision

Cardiac rehabilitation (CR) is the treatment option that can reduce the likelihood of recurrent events and disease progression, and should incorporate all the secondary prevention strategies currently available for patients at high risk of cardiovascular events.

The changing emphasis on providing care closer to the patient has meant that cardiac rehabilitation provision is increasingly being provided in primary care at local community venues. This paper will review current literature, need and cost efficacy of providing this form of secondary prevention and take a look at how one Primary Care Trust has set up a comprehensive service, designed to enhance recovery following a cardiac event or surgery.
